Program guidelines

 

1. The first 50 small businesses to request a quote will receive a $50 gift card from one of the following businesses: Starbucks, Panera Bread, Home Depot or Lowe's.

3. Applies to new business only. One gift card per business.
    
4. All winners will receive their gift cards within 30 business days after quote is provided.
    
5. All winners will be notified via email or phone to provide a Virginia mailing address or email address for the gift card to be sent to.
    
6. Campaign runs through October 31, 2021 for the first 50 businesses to request a quote.

 

Who's eligible?
 

To qualify for the offer, a business must:
 

1. Be headquartered in Virginia
    
2. Not be a current Delta Dental group customer
    
3. Request and receive a quote for a Delta Dental of Virginia group dental plan, and
    
4. Provide a valid business email and phone number.

Disclosures
 

1.  To the extent applicable, payments received from this program may affect your IRS Form 1099.
    
2. To the extent applicable, supplemental bonuses paid pursuant to this program will be reported in accordance with the IRS Form 5500 instructions.
    
3. Offer is subject to Virginia law, and recipient is responsible for all applicable laws.
    
4. Offer not transferable except at Delta Dental of Virginia’s sole discretion.
    
5. The program is subject to modification at any time at the sole discretion of Delta Dental.
