1. Nominations for the office manager sweepstakes must be made solely through the Delta Dental of Virginia website between September 26, 2022 and October 7, 2022. We cannot accept emailed nominations at this time. All winners will be notified via email or phone to provide a Virginia mailing address or email address for the Gift Card to be sent to.
   
   
   • Five winners will be chosen at random from the submitted nominations. Gift Cards are limited to one winner per Sweepstakes submission.
      
   • No purchase or other consideration necessary for entry. Void where prohibited.
      
   • Winners will be asked to provide written consent to post/publicize their story, name and/or photo on Delta Dental of Virginia's website and/or social media pages. Winners are not required to provide consent in order to win their prize.
      
2. Winners will receive a $50 Amazon Gift Card. 
    
3. All winners will receive their Amazon Gift Cards within 2-3 weeks of award notification.
    
4. Winning dental office staff members must currently work in a practice located in the state of Virginia.

6. Campaign runs through October 7, 2022 to allow for up to five $50 winners. Five dental hygienists will be selected.
