1. Nominations for the dentist and dental assistant contest must be made solely through the Delta Dental of Virginia website until March 31, 2022. We cannot accept emailed nominations at this time. All winners will be notified via email or phone to provide a Virginia mailing address or email address for the Gift Card to be sent to.
   
   
   • Winners will be chosen at random from the submitted nominations. Gift Cards are limited to one winner per Sweepstakes submission.
      
   • No purchase or other consideration necessary for entry. Void where prohibited.
      
   • Winners will be asked to provide written consent to post/publicize their story, name and/or photo on Delta Dental of Virginia's website and/or social media pages. Winners are not required to provide consent in order to win their prize.
      
2. Winners will receive a $50 Amazon Gift Card. 
    
3. All winners will receive their Amazon Gift Cards within 2-3 weeks of award notification.
    
4. Winning dental office staff members must currently work in a practice located in the state of Virginia.

6. Campaign runs through March 31, 2022 to allow for up to ten $50 winners. Five dentists and five dental assistants will be selected. 

Each winner is eligible to win one prize of a $50 gift card.
